Form R
Each year, as a trainee, you will receive an email with a link to fill in your Form R. This important document must be completed and submitted as part of your ARCP. Please follow the instructions enclosed in the email carefully.
This form is currently hosted on the HEE survey system. Due to the UK GDPR, you must download and save your responses as a PDF within 15 minutes – as after that time your responses will be deleted. It is this PDF that you must upload into that year’s ARCP folder on your Kazien ePortfolio (not the “certificate” that is offered for download at the submission confirmation screen).
